Retrospective U.S. death certificate analysis (CDC WONDER, adults ≥25 years, 1999–2024) found 31,497 deaths mentioning both pancreatic cancer and renal failure. Age-adjusted mortality rates rose from 0.37 to 0.82 per 100,000, with the steepest rise during 2018–2024 (annual percentage change 10.77%; 95% CI 7.18–18.22%). Renal failure was co-recorded in 2.98% of pancreatic-cancer-involving deaths vs 2.37% (lung/bronchus), 4.75% (colorectal), and 5.77% (liver/intrahepatic bile duct); recent trends did not significantly differ from pancreatic cancer. Acute kidney failure was the most frequent subtype (43.22%). In 2024, rates were higher in males and highest in non-Hispanic Black individuals. Death-certificate data lack temporal sequence/causality; linked clinical data are needed.
